As a member of the Community Care Case Management Team you will be responsible for the assessment, design, and implementation of continuing care services for a caseload of clients residing in the community. You will also be responsible, as a part of the team, for the provision of Registered Nurse consultation, assessment, care, and health teaching to clients in various community living settings as required related to your caseload and your team's needs. Demonstrated Knowledge, Skills and Abilities in: Comprehensive assessment, case management, and intervention techniques. Current trends, issues and practices in community care. Community resources. Current best practices in case management processes. Understanding of Quality Improvement theory and methodologies. Conflict resolution and crisis intervention. Leadership. Time management and organization. Effective oral and written correspondence . Working independently and as a member of a team. Working in a quick paced, ever changing environment. Working with people in health care environment with professionalism and respect. Positive approach and good listening skills in dealing with challenging situations with empathy and sincerity.Required Active CARNA registration or eligible for same. Baccalaureate degree in nursing preferred; however, skills and background relevant to nursing practice in community sector will be considered. Preference given to applicants with Community Care Case Managerial experience. Recent clinical background in psychiatric and geriatric nursing preferred. Knowledge of and skills in applying Home Care Nursing Practice Standards. Negotiating skills with clients, caregivers and families. Family assessment skills. Knowledge of Guidelines for Assignment of Health Care Procedures by Health Care Aides. Knowledge of Alberta Aids to Daily Living (AADL) Program is desired. Understanding of principles of Palliative Care preferred. Community Care provides a comprehensive orientation. Suitable applicants must be available to attend the orientation.
